Accountable Communities of Health (ACHs) are the Healthier Washington Mechanism for Community Empowerment and Accountability. ACH's convene communities and bring together social sectors, such as public health, the health care delivery system and healthcare providers and the system which make up the social determinants of health. Important factors of success of ACHs include a strong foundation that is respected and accountable with broad community support to the ACH concept.
As a deliverable of the Technical Assistance Contract with the Washington State Health Care Authority and Health Philanthropy Partnership, Empire Health Foundation is contracted to develop a suite of Technical Assistance services and resources to support the vision of an optimally functioning ACH. This blog will function as the ACH Toolkit, it will operate as one location for ACHs to find important materials, collaborate, learn and ultimately move the dial on health in our state.
Navigating the Site:
The blog is made to make finding information for the Accountable Communities of Health convenient and accessible. In addition to blog posts with ACH Toolkit information and experiences from ACHs as they implement such tools the blog will also act as a central place to find best practices from around the country.
The ACH Toolkit tab will lead you to find more ACH Technical Assistance documents and blogs. You will be able to search by a particular topic for instance Governance, Sustainability, and Community Engagement.
The Resources tab will be a central location where ACH's can find best practices from around the country and from other ACH's in the state as they implement similar practices and any best practices they have developed.
