Building Your Project Portfolio 

This webinar was presented by Healthier Washington to present Phase II Certification Transformation Project Planning Criteria.  The webinar provides a summary of the anticipated projects and how ACH are approaching or intersecting across anticipated projects in support of a project approach. (Updated July 17th, 2017)

This current version of the DSRIP evidence catalog provides basic information about DSRIP-related articles, reports and presentations and which projects the resources are most applicable for. 

Implementation Approach and Timing -- Considerations for Developing and Actionable Plan 

This document provides ACHs with a set of considerations to inform their DY 1 planning and development of their Implementation Approach and Timing subsection of the Project Plan Template.  These considerations provide a framework which will need to be tailored to address the unique aspects of each project, particularly as ACHs finalize their full implementation plans by DY2 Q3. Implementation Approach and Timing -- Example Narrative

This document provides an example narrative for the Implementation Approach and Timing section of the Project Plan Template. This example does not represent an “ideal” response, rather it should be considered a framework that highlights the level of detail and tactical steps needed in a successful response.  This example references a fictional ACH called “TPA”, and any counties, organizations and partners referred to in this are fictional and included solely to support the narrative. Phase Certification II Webinar

In June 2017, Manatt gave a webinar as the DSRIP Technical Assistance contractor for Healthier Washington on the Certification Phase II Submission.  The webinar gives a debrief on the Phase I Certification and discussion of the changes for Phase II Certification and Submission Template. 

Project Plan Review Tool

HCA has engaged Myers and Stauffer LC to serve as the Delivery System Reform Incentive Program (DSRIP) Independent Assessor for the Medicaid Transformation Demonstration. Myers and Stauffer will conduct assessments of Project Plans submitted by Accountable Communities of Health on November 16, 2017. We are seeking your comments on two components of Myers and Stauffer’s Project Plan review tool: the evaluation criteria/definitions and scoring methodology. (September 22, 2017)

Project Plan Template Webinar

This webinar overviews the final Medicaid Transformation Demonstration Project Plan Template (released 7/30/17). It provides an overview of the template, deep dives on Section I (ACH-level) and Section II (Project-level), and preliminary project plan scoring. 

Project Plan Template (updated Oct. 20)

Adjustments have been made to the Domain 1 narrative in Project Plan to clarify that:

  • Activities should focus on regional planning and capabilities directly related to each ACHs’ selected projects;
  • and Responses can reflect ACHs’ preliminary thinking.
  • Other adjustments reflect the narrative on the required percentage funding distribution projections by “use category” and “organization type” in the Project Plan workbook.

Project Plan Supplemental Workbook (updated Oct. 20): 

This document has been revised to accommodate a number of requests:

  • Information related to Integration incentives have been removed and clarification in the template. This was a request from a recent office hours discussion.
  • Use categories have been revised. However, please note that it is expected that ACH provide a definition and percentage for these required use categories.
  • The first funds tab requests a combined 5-year projected percent breakdown of Project Incentive funds by use category, with clearer definitions and without associated projected dollar values.
  • The second funds tab is still required for DY1 projections.
  • Partnering provider tabs have been revised (see #3)

Project Plan Submission Amendment Project Plan Withdrawal Form

This form is for use by the ACH when they decide to drop more than one project by the January 31, 2018 deadline. Changes to the approved ACH Project Plan after the Project Plan Submission Amendment deadline will require a formal Project Plan modification, and subject to a separate process. (November 13, 2017)

Regional Assets, Anticipated Challenges and Proposed Solutions

This document includes:  Potential regional assets – Potential expertise, capabilities, systems, and other resources that ACHs may identify in their own communities and consider leveraging for specific project implementation and possible ACH challenges and mitigation strategies – A matrix describing potential barriers to implementing projects and achieving outcomes and total cost of care improvement goals for target populations. The matrix lays out mitigation strategies that ACHs may consider adopting to overcome these potential project barriers. (September 8, 2017)


Secondary workbook with Provider details   

To separate the administrative aspects of the partnering provider tabs, a second workbook has been created. While ACHs must provide a list of partnering providers for the Nov 16th submission, provider details, including EIN, can be submitted separately. (October 20, 2017)

  • This secondary workbook is due December 15th.The format of this workbook is a single provider list where multiple projects can be selected for each partnering provider.  
  • This workbook requires each ACH to indicate which partnering providers should or should not be added to the Financial Executor portal. It is expected that the majority of identified providers will be marked “Y” to support uploads/registrations.  This is for registration purposes only and does not dictate ACH funds flow arrangements or guarantee future payment.

Theory of Action & Project Plan Webinar

The following resource is a webinar given on June 5th, 2017 regarding the Theory of Action and Project Plan Template. This resource gives ACHs exercises that can support a strong Theory of Action and Alignment Strategy and examines Project Plan Template, goals, timelines and overview.


Target Population Clarification

This document is intended to clarify project plan expectations and offer examples of narrative related to target populations.  Please note that these examples from the NY DSRIP address the anticipated target population but not the anticipated number of individuals reached through selected projects. A complete answer from an ACH must address both elements to receive a full score. (October 20, 2017)


Washington CPAS User Reference Guide

This is the Washington Collaboration, Performance and Analytics System (CPAS) user reference guide. This document includes guidance on registering for a new CPAS account; how to upload and download files on the ACH Document Repository Page; and information on the WA DSRIP Help Desk.
